Case Management
This department is comprised of nurses and social workers who collaborate with other health care providers to ensure that services which best meet the needs of each patient are timely and available. Staff assists patients and families in exploring options for follow-up care post-hospitalization including both short-term rehabilitation, long-term placements, and hospice care. In addition, the department is a resource to the community for information on support groups, health care proxies, and many other health related services. Call 508-236-7680 for more information.
